Annual Membership runs from January to January and costs just £8 for you and a partner.

If you would like to pay by cheque or cash, please complete the form and print it (or print it blank and fill it in by hand) and post it along with your payment to PHSG Office, 88 Puddletown Crescent, Poole, BH17 8AN.

When we receive your details and payment, we will send you your Membership Card, a copy of our latest magazine and a form that your doctor needs to sign before you can excercise. Please bring the completed form to your first Exercise Session along with your Membership Card which you should bring to every session.
